Emergency Support Request
Please use this form only in Emergency Situations
The following situations qualify for Emergency Support:
- Entire System Down
- Unable to take payment
- Cannot log into System Five
- Recovering from a Power Outage
Be as descriptive as possible so we can resolve your problem efficiently.
- Upon successfully clicking the "SEND" button on the emergency form, you will receive an email from our ticketing system with a tracking number.
- You can sign in to the customer care portal to add additional information or track your ticket's progress.
